Mesothelioma is frequently framed as a legacy ailment-a remnant of an industrial era when asbestos was ubiquitous in construction, shipbuilding, and manufacturing. However, a comprehensive analysis from the Sylvester Comprehensive Cancer Center at the University of Miami Miller School of Medicine reveals that the disease is not fading. While age-standardized rates of incidence and mortality have declined, the total number of people suffering from the cancer is increasing.

This divergence is driven by an aging U.S. population and the prolonged latency period of the disease, where exposure decades ago manifests as malignancy today. It also underscores a central tension for health planners and regulators: progress on paper does not automatically translate into a lighter burden for hospitals, compensation systems, or public budgets.

“Mesothelioma behaves like a time capsule. What we’re seeing today reflects exposures that happened 30 or 40 years ago, and in some cases, exposures that are still happening in quieter, less visible ways,” says Chinmay Jani, M.D., chief fellow at Sylvester and co-lead author of the study.

Kyle Edwards, co-lead author and M.D./M.P.H. student at the University of Miami Miller School of Medicine, emphasizes the public health implication: “Declining rates don’t necessarily mean fewer lives affected. When you look at absolute numbers and disability, mesothelioma remains a substantial public health problem.”

Divergent Risk Profiles by Gender

The data indicates a widening gap in how mesothelioma affects men and women. For men, the decline in incidence and mortality is sharp, largely because the cohorts most heavily exposed in shipyards and construction are aging out and because certain high-risk workplaces are now more tightly regulated. Women, however, are not seeing the same relief.

In 20 states, female incidence increased, and in 18 states, female mortality rose, with notable concentrations in Appalachia and the Midwest. This suggests that women are being exposed through pathways that bypass traditional industrial employment and, by extension, some of the formal occupational protections designed for high‑risk workers.

Exposure vectors for women include:

  • Para-occupational exposure: Inhaling asbestos fibers brought into the home on the clothing and skin of working family members.
  • Environmental contamination: Proximity to asbestos-mining sites, legacy industrial zones or contaminated soil and dust.
  • Legacy infrastructure: Exposure during renovations or through deteriorating materials in schools, homes and public buildings, often outside formal workplace safety oversight.

“For women, mesothelioma often doesn’t come with a neat occupational history,” says Estelamari Rodriguez, M.D., M.P.H., clinical research lead of the Thoracic Oncology Site Disease Group at Sylvester. “Environmental exposure, para‑occupational exposure from family members and legacy asbestos in schools, homes and public buildings likely play a much larger role.”

For policymakers, this gender divergence complicates traditional worker-focused approaches. Protecting women and children from para‑occupational and environmental exposure demands closer coordination between labor regulators, housing authorities and local school districts.

Geographic Clusters and Environmental Legacy

The distribution of mesothelioma is not uniform across the United States; it is instead a “patchwork quilt” of risk dictated by regional industry, infrastructure age and local geology. Certain states continue to report disproportionately high burdens of disease, shaping where health systems, environmental agencies and state legislators face the greatest pressure to act.

High-burden regions and their primary drivers:

  • Maine, Washington: Heavy historical reliance on shipbuilding and naval infrastructure, with asbestos used extensively in ships and coastal facilities.
  • Alaska: Disturbance of naturally occurring asbestos during construction, road building and infrastructure development in remote terrain.
  • Minnesota: Mineral fibers associated with legacy iron ore mining operations and related industrial activity.

“Mesothelioma is a geographic disease,” Jani says. “State‑level data allow us to see where prevention, remediation and surveillance efforts are most urgently needed.”

For state and local governments, that means decisions about school renovations, public housing upgrades and industrial clean‑ups are not merely infrastructure choices-they are long‑tail cancer‑prevention measures that may only show impact decades from now.

Stagnant Survival Rates and Therapeutic Limits

Despite the introduction of newer pharmacological interventions, including immune checkpoint inhibitors, the population-level survival rate for mesothelioma has remained stagnant for over three decades. The mortality-to-incidence ratio remains near 1, meaning the vast majority of patients still succumb to the disease.

This suggests that while new therapies may extend life for some individuals, they have not yet shifted the overall trajectory of the disease on a national scale. For health systems and payers weighing expensive oncology innovations, mesothelioma still behaves less like a chronic condition to be managed and more like a rapidly terminal cancer that demands earlier detection and primary prevention.

“From a patient’s perspective, this is the hardest message,” says Gilberto Lopes, M.D., Sylvester’s chief of medical oncology. “Despite all our scientific advances, mesothelioma remains one of the most lethal solid tumors.”

Regulatory Gaps and the Remediation Challenge

The persistence of mesothelioma is a direct consequence of regulatory history. Unlike many other developed nations, the United States has never enacted a comprehensive federal ban on asbestos. Instead, the Environmental Protection Agency (EPA) and other federal agencies have relied on a complex web of partial bans, material‑specific rules and workplace standards that leave significant amounts of the mineral in place across the built environment.

Under frameworks such as the Toxic Substances Control Act, regulators have progressively constrained new uses of asbestos, but they have not mandated the systematic removal of what is already embedded in pipes, insulation, ceiling tiles and other materials in older buildings. That distinction between restricting future use and funding large‑scale remediation continues to shape the country’s mesothelioma burden.

The study confirms that nearly 96% of U.S. mesothelioma deaths in 2023 were attributable to occupational asbestos exposure, a figure that has remained constant since 1990. This highlights the danger of asbestos embedded in aging infrastructure, where demolition, renovation and disaster damage can re-release fibers into the air and expose workers, residents and first responders.

The findings, published in JCO Global Oncology, argue that a dual strategy of aggressive remediation and investment in therapeutic breakthroughs is the only way to reduce the future burden. For Congress, federal agencies and state legislatures, that translates into decisions on whether to dedicate sustained funding to asbestos abatement in schools and public housing while also backing clinical trials and early‑detection research.

“Mesothelioma reminds us that prevention and treatment are inseparable,” says Lopes. “We have to keep people from being exposed in the first place, while also doing better for those who are diagnosed.”

Jani concludes that the long-term nature of the disease requires a permanent commitment to surveillance. “Mesothelioma reminds us that the consequences of exposure can last a lifetime-but so can our responsibility to patients. By understanding where this disease still concentrates and who it continues to affect, we move closer to a future where prevention is stronger, treatments are more effective, and no community is left behind.”
